New Patient Policy:

• We welcome new patients of all ages to our practice.

• New patients are required to complete a comprehensive health history form prior to their first appointment.

• New patients are required to forego an in-depth health history and a comprehensive physical exam on their first visit.

 Appointment Scheduling:

• Appointments can be scheduled by calling our office during regular business hours or by emailing the office.

• We strive to accommodate urgent appointments as soon as possible.

Cancellation Policy:

• We understand that circumstances may arise that require you to cancel or reschedule your appointment.

• We kindly ask for a minimum of 24 hours' notice for cancellations or rescheduling.

• Missed appointments or late cancellations may be subject to a fee of $25

Payment Policy:

• Payment is due at the time of service. We accept cash and credit/debit cards.

• We are happy to provide you with a detailed receipt/superbill that you can submit to your insurance company for reimbursement.

Insurance:

• We are a cash-based chiropractic office and do not accept insurance at this time.

Privacy Policy:

• Your privacy is important to us. We adhere to strict confidentiality guidelines and will not disclose your personal information without your written consent.

• Patient records are securely stored and accessible only to authorized personnel.

Emergency Care:

• In the event of a medical emergency, please call 911 or visit the nearest emergency room.
